This

rehabilitation organizations, and guardians that deliver care to auto accident
victims.
You are most likely aware of case managers and the services they
provide to auto accident victims.
Here
is a fact that you may not be aware of: Under the Auto No-Fault
law in Michigan the victim has a right to choose their own case
manager. If your patient has already been assigned a case manager
from the insurance company, he/she maintains the right to change case managers. If
your patient has not been assigned a case manager and you think they
need one either the patient or yourself can contact us.
Benefits of CareForward Case Management:
- We are a group of case managers that take referrals only from
providers, such as yourself.
- We have no obligation other than to provide the best-case management
available for your patient. - If we receive a referral from you,
we contact the client set up an appointment and allow them to interview
us. Then they choose if they wish to work with us.
- We attend all doctor appointments related to the auto accident,
with your patient.
- We can assure that the patient attends appointments and follows
through with the plan of care.
- We can arrange for transportation if necessary.
- We initially attend therapy appointments and ensure that care
is coordinated and encourage case conferences if necessary to provide
continuity of care.
- We work hand in hand with internal case managers to ensure progress
throughout the continuum of care.
- We will provide orders written by the physician to the appropriate
providers of care.
- If you have concerns regarding the progress of the patient we
are available for consult or conferencing to address problems.
- And best of all you will not need to argue with us as to why
your treatment is needed. If it is best for the client, we are right
behind you.
- We work with you to show the insurance company why your services
are "reasonable" and "necessary": fulfilling
the requirement stated in the auto no fault law.
- We will not work with patients that are abusing the system.
- Our goal is to make your treatment of the patient
successful.

When you send us a referral:
- We will set up an appointment to meet them and their family and/or
caregivers, usually in their home. They will be given information about us
and meet the case manager assigned to them.
- We will interview them to see if we feel that case management is
reasonable and necessary.
- If the client chooses to use our services we will open the case and provide
services.
