You may have little to no knowledge about how to help yourself or another through this process. Some of you may have "been through the mill" and are looking for more resources or options to better manage your care.

The following information is for Michigan residents with Michigan auto policies only and not meant to provide legal advice - I am not a lawyer. If you need legal support please contact an attorney. First = If you have been injured due to an auto accident you may qualify to receive case management services. This is part of the medical benefit coverage available through Michigan automobile policies. THIS COVERAGE IS FOR 100% PAYMENT OF SERVICES.

Second = The insurance company may have already assigned you a case manager. It is important to understand that you have the right to choose you own case manager.

Third = CareForward is a group of case mangers who take referrals from the survivor of auto accidents, their family and/or providers of care. We understand that you have the right to hire us or fire us. IT IS YOUR CHOICE to work with us.

Fourth = We will set up a time to meet you in your home or at your bedside. You may ask any questions of us in the privacy of your home. We will give you information and explain what we have to offer. Then you can choose whether our services would benefit you.

Fifth = If you choose to work with us we will begin gathering information from you and your medical team to assess how we can assist you through your process of recovery.
